Warning: The participant is obliged to provide information in accordance with Act No. 362/2011 Coll. on Medicinal Products and Medical Devices, as amended by subsequent regulations, accurately, completely, and truthfully, and bears full responsibility for their correctness, accuracy, completeness, and truthfulness.

Consent of the concerned person: In accordance with § 14 of Act No. 18/2018 Coll. on the Protection of Personal Data and on the Amendment and Supplementation of Certain Acts, I freely and voluntarily consent to the processing of the personal data provided in this registration form by the Slovak Medical Society (SLS) and its organizational components for the purpose of my participation in the specified professional event.

Notification: Personal data will be processed to ensure the administration and other requirements related to your participation in the professional event registered in the CME Event Calendar. SLS is the controller of personal data processing. The legal basis for processing is the consent you have given. The consent is voluntary and can be withdrawn at any time. In the event of withdrawal of consent prior to the professional event, it will not be possible to participate. Personal data will be retained for up to 5 years from the end of the professional event. The participant has the right to access the data, rectify them, erase them, restrict processing, and object to the processing (§ 21 to 28 of the forementioned Act).
