It is possible to apply to the Slovak Society of Oromaxillofacial Surgery (SSOMFCH) as a subsidiary professional society of the Slovak Medical Society (SLS) by filling out the form.
In addition to personal data, it is necessary to state the name of the professional company and its numerical code:
"Slovak Society of Oro-Maxillo-Facial Surgery (SSOMFCH) - code 75"
and the name of the medical association and numerical code according to the appendix.
The completed application must be sent to the address:
