It is possible to apply to the Slovak Society of Oromaxillofacial Surgery (SSOMFCH) as a subsidiary professional society of the Slovak Medical Society (SLS) by filling out the form.
Okrem osobných údajov je potrebné uviesť v Prihláške – II. časť – name of the professional company and its numerical code:
"Slovak Society of Oro-Maxillo-Facial Surgery (SSOMFCH) - code 75"
and the name of the medical association a číselný kód podľa prílohy (strana č. 5).
